Admission-
We at Touching Heart's Group Homes, LLC admission procedure is to be fast and easy. Support Coordinator agencies will provide admission screening within 24-48 hours of inquiry. It's our task at Touching Heart's to respond to the support coordinator with current status on bed availabilities.
Transition-
We recognize there are barriers and that everyone's independence level is different; but our number one goal is to focus on improving and attaining skills. Success can be achieved at any level. the following services are provided:
•Self-Help needs (i.e. bathing, dressing, medications, etc)
•Independent living needs (i.e. meal planning and preparation, housekeeping, laundry etc.);
•Ambulation and mobility;
•Securing transportation; and
Safety and security needs.
Resident Waiver Support Coordinator will be notified with a written transitional letter stating what the consumer request is about moving, or, lease being terminated. Touching Heart’s will make sure the resident transitional is smooth in easy. Once a new home is located for the resident, life book, medical book, medications, resident funding (if any) and the resident clothing will be given to the new provider with a signed released sheet for the items. If Resident requests to relocate to another group home or the agreement is terminated for any other reason, Touching Heart’s will continue to provide quality services to the Resident until the transition to the new provider happens. Touching Heart’s will work with all Parties of this Agreement and the APD Regional Office to make sure the Resident’s transition to and from another residential habilitation service provider is smooth (if desired by the Resident or their Representative). All records and property will be forwarded in a timely fashion to avoid delays in service.
Discharged
Each individual/guardian have every right to discharge at anytime. Touching Heart's Group Homes, LLC will make sure each request is granted with understanding; making discharge comfortable and easy for ALL individuals/guardians.
